WHAT TO EXPECT DURING A CONTACT LENS EXAM:
There is a lot more to contact lens wear than meets the eye! The contact lens has to have the correct power, which may or may not be the same power of the lens in your glasses. If you have astigmatism, the amount and orientation need to be factored into the fitting process. The contacts need to properly fit the surface of the eye. They cannot move too much or too little. Either one could result in discomfort, pain or even damage to the eye. Learning to insert and remove a contact lens is an important part of the fitting process. A contact lens fitting requires extra time and therefore a separate appointment.

Dry Eye Disease: There are two main categories of dry eye. Both result in discomfort, burning, gritty sensation and often blurred vision which tends to fluctuate with blinking. Aqueous tear-deficient dry eye is a disorder in which the lacrimal glands fail to produce enough of the water component of tears to maintain a healthy eye surface. Evaporative dry eye may result from inflammation or poor function of the Meibomian glands located near the eyelashes. The resulting lack of the oil component of tears results in rapid evaporation from the eye surface. Artificial tears just don’t do enough for this type of dry eye. New treatments are designed to improve the production of natural oil by the glands.

Recent research indicates that the vast majority of dry eye patients suffer from rapid evaporation of tears rather than insufficient production of tears. This is known as "evaporative dry eye". It is the result of dysfunction of the many glands that are close to the eyelashes. "Meibomian gland dysfunction", as it is called, results from unstable tears that evaporate much too quickly. Even frequent use of artificial tears cannot keep pace with the evaporation. The result is dry eye symptoms: burning, scratchy feeling, redness, eye fatigue, and vision which seems to change constantly with blinking or rubbing. Expensive prescription medications may help increase the amount of tears but do nothing for evaporative dry eye. Acne rosacea, blepharitis, skin conditions and age can all be associated with poor meibomian gland function leading to evaporative dry eye. Ocular rosacea, blepharitis, and meibomian gland dysfunction can be diagnosed during the eye exam. An in-office treatment, known as "intense pulse light" or IPL, can treat underlying conditions and greatly improve the symptoms of evaporative dry eye. Prescription medications may no longer be necessary. The need to purchase, carry and instill artificial tears is usually significantly reduced.

In cataract surgery, the lens inside your eye that has become cloudy is removed and replaced with an artificial lens (called an intraocular lens, or lens implant) to restore clear vision.

Glaucoma is a disease that damages the optic nerve. The pressure inside the eye becomes elevated. There are usually no symptoms. The only way to prevent nerve damage and possibly blindness is to have the pressure checked as part of an annual exam. Most glaucoma patients are treated with medication in the form of eyedrops. However, there are newer treatments which allow for control of eye pressure with fewer or possibly no drops.

Age-related macular degeneration (AMD) occurs when there is deterioration of the central portion of the retina, knows as the macula. The ability to see details and colors comes from this special part of the retina. AMD is common and occurs mostly after fifty years of age. There are two types of AMD. About 80% of AMD is classified as "dry". Dry AMD usually progresses slowly with gradual loss of central, sharp vision. The peripheral/side vision remains intact. "Wet" macular degeneration is the result of new blood vessels which leak fluid or bleed into the macula, causing rapid loss of central vision.

Risk factors for AMD include high blood pressure, high cholesterol, poor nutrition, cigarette smoking, ultra-violet light from the sun, and family history. Yearly eye exams with dilation of the pupils are recommended for early detection of macular degeneration. If detected early, progression of dry AMD can be slowed with special vitamin supplements and changes in lifestyle. Wet AMD, if detected early, can be treated by a retina specialist with injections into the eye to reduce the number of abnormal vessels that are leaking or bleeding.
